Families with participating BCBS, Anthem, Cigna, United, and Humana PPO's, as well as plans displaying the PNOA or Multiplan Logo (many UHC and Aetna policies) on the back of the card, may qualify for lactation office consultations at no out-of-pocket cost. Home visits will incur a travel and set up fee that is not covered by insurance. Virtual consultations require a one time video pre-assessment fee that insurance doesn't cover. The Affordable Care Act mandates that lactation services are covered. If your insurance is not accepted as in-network by Lactation Network, a Superbill will be provided for you to submit to your insurance for full or partial reimbursement based on your plan. If you are a Tricare or Medicaid subscriber, please contact Amy for special rates and booking for in-office or virtual services only.

In-Office Consultation
This friendly and comprehensive private consultation in our cozy office focuses on a detailed history of your pregnancy, birth and feeding experience including breast exam, infant oral and anatomical examination, comprehensive feeding assessment, a personalized care plan centered on your feeding goals and communication with your health care providers. Two weeks of messaging with the IBCLC through our secure portal are included in your consultation fee. We have office hours Tuesday, Wednesday, Thursday and selective Saturdays.

VIRTUAL CONSULTATION
Community Lactation and Parenting offers virtual consultations as a way for breastfeeding families to get expert clinical support and information in the comfort of their own homes. A comprehensive history of your pregnancy, birth and feeding history will be collected prior to our appointment. Assessment of positioning and latch will be performed via video (it's helpful for you to have a support person available to help). We will work together to develop a care plan based on your feeding goals. A detailed report will be sent to the physicians involved in the care of both breastfeeding parent and baby and a superbill will be provided for you to submit to insurance for potential reimbursement. Two weeks of secure messaging with the IBCLC through our secure chat portal is included in your consultation fee.
(Time overages will be billed $20 per 10 minute increment. Tricare and Medicaid subscribers are eligible for discounted rates. Please contact Amy for details.)

In-Home Consultation
These relaxed consultations allow for skilled, hands-on help in the privacy and comfort of your own home. They usually last about 2 hours and include a detailed history of your pregnancy, birth and feeding experience, including breast exam, infant oral and anatomical examination, comprehensive feeding assessment, a personalized care plan centered on your feeding goals, communication with your health care providers and an insurance superbill for self-pay clients to submit to insurance for reimbursement. Two weeks of secure messaging follow-up included. Availability for travel outside of Yulee/Fernandina may be limited during high volume.
