Home visits will incur a travel and set up fee that is not reimbursable by insurance. Insurance may reimburse you for the time that we are together, but not travel/commute and set up time.
The Affordable Care Act (ACA) mandates that lactation services are covered. A superbill receipt will be provided for you to submit to your insurance for full or partial reimbursement based on your plan. We will provide relevant ICD.10 (diagnosis) and CPT (procedure) codes as well as NPI (National Provider ID) and EIN (tax ID) required for reimbursement. It is recommended to call your plan’s Member Services line (number found on the back of your card) ahead of our visit, text me at (904)321-9098 for information and scripting prior to your call with your insurance carrier.

In-Office Consultation
initial: $250
follow ups: $195
This friendly and comprehensive private consultation in our cozy office focuses on a detailed history of your pregnancy, birth and feeding experience including breast exam, infant oral and anatomical examination, comprehensive feeding assessment, a personalized care plan centered on your feeding goals and communication with your health care providers.
VIRTUAL CONSULTATION
initial: $250
follow up: $100 per 30 min.
Community Lactation and Parenting offers virtual consultations as a way for breastfeeding families to get expert clinical support and information in the comfort of their own homes. A comprehensive history of your pregnancy, birth and feeding history will be collected prior to our appointment. Based on your case, I will ask for you to capture some videos and photos of your baby for me to assess in advance of our visit. We will work together to develop a care plan based on your feeding goals. A detailed report will be sent to the physicians involved in the care of both breastfeeding parent and baby. A superbill will be provided for you to submit to insurance for potential reimbursement .
In-Home Consultation
Initial: $325
follow ups: $275
These relaxed consultations allow for skilled, hands-on help in the privacy and comfort of your own home. They usually last about 1.5 to 2 hours and include a detailed history of your pregnancy, birth and feeding experience, including breast exam, infant oral and anatomical examination, comprehensive feeding assessment, a personalized care plan centered on your feeding goals, communication with your health care providers and an insurance superbill to submit to insurance for reimbursement. Availability for travel outside of Yulee/Fernandina may be limited during high volume.
(Travel fee varies based on mileage from your consultant’s home.)
