1. Once you have scheduled your appointment and determined insurance coverage, please accept the Google Meet invitation you received via email. 

2. Using the button below, complete the Lactation Intake Forms via the Mobile Lactation Consultant app that we use to securely store your health information.

3. Prior to your scheduled time, there are a few videos and photos of your child that I would like to view prior to or during our consultation. You will find instructions for submitting these to me by viewing the guide below.

4. Before our scheduled time, make sure your computer/mobile device is ready! Use the guide below for instructions!
